Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send them to the address below no later than April 2, 2026.
- Duly accomplished Personal Data Sheet (or CSC Form 12, series of 2017) with recent passport size picture with name and signature;
- Photocopy of Transcript of Records;
- Photocopy of certificates of seminars and trainings attended (not later than 5 years);
- Photocopy of certificate of employment (if applicable); and
- Photocopy of certificate of eligibility/rating.
Applications with incomplete documents shall not be entertained.
